After entering your acres into the on line database, attach the following to your signed contract and certification page:

• Most recent FSA Form 578 Report of Commodities (Farm and Tract Detail Listing) for all acres enrolled.
• Most recent FSA Form 578 Report of Commodities (Farm Summary) for all farms enrolled.
• Most recent Aerial Maps for all parcels enrolled. Maps must be originals or clear copies
  Maps MUST be marked with:
  
  • Farm and Tract numbers
  • Acres in each tract
  • Legal Description of mapped areas

**Incomplete applications and those received after the pool deadline may not be considered.**

All documentation may be submitted electronically via email to carboncredit@ndfu.org. Be sure your contract number is in the email subject as well as on all attachments. Signed original contracts must be submitted via regular paper mail. In addition to shapefiles, acceptable file formats include: .pdf, .jpg, .xls, .xlsx, .tif, .bmp, .doc, .docx, .txt, .rtf, .csv.
